Case Manager II

Transitions works to end poverty and homelessness for women and children in Spokane.

We have a current opening for a Case Manager at Sinto Commons. This position works on-site in a residential apartment building 40 hours/week. In keeping with Transitions’ mission and core values, this position supports single individuals and families who have transitioned out of homelessness and are working toward housing stabilization and resolving barriers that lead to homelessness.

You may be well-suited for a role as a PSH Case Manager—helping residents transition from homelessness to stability with compassion and expertise—if you are a person who is skilled in the following:

1.     Diversity and Inclusion: Willingness to work with families of all compositions, ages, generations, and genders, demonstrating an inclusive approach.

2.     Community Building: Ability and enjoyment in building community and engaging with residents both individually and in community settings.

3.     Trauma-Informed Approach: Skills in working through a trauma-informed lens, understanding the impact of trauma on individuals and communities, and adapting practices accordingly.

4.     Passion for Housing First: Strong commitment and passion for solving homelessness in Spokane through the Housing First model, which prioritizes providing stable housing as a first step.

5.     On-Site Service Provision: Experience or readiness to provide services directly on-site in an apartment building setting, ensuring accessibility and convenience for residents.
